The photograph above shows a large gearbox that suffered a catastrophic failure. The gears were splash lubricated from the 24.2 gallon sump and the bearings were fed via pipe work from an externally mounted electric motor and pump. The vertically mounted driving pinion gear, shown in the photograph below, was almost completely stripped of its teeth, and the mating gear set showed severe burring on the leading edges where the metal had been mechanically displaced by heat.

The cause of this failure turned out to be low oil level. The photograph below shows the bottom half of the gearbox with the arrow marking the approximate position of the correct oil level, measured by a dipstick when the gearbox was inoperative. The position X depicts the bottom of the lowest gear, and the position Y was the actual level of the oil. A thorough investigation took place and a full report was issued detailing the causes of the failure, the current lubricating regime, as well as recommendations covering lubrication, maintenance schedules, and operating procedures. This failure cost a large sum of money in lost production alone.
